A painful lump near your tailbone can make sitting, studying, driving or working uncomfortable. In young adults, one possible cause is a pilonidal sinus, a small opening or tunnel beneath the skin near the top of the buttock crease.

What Is a Pilonidal Sinus?

A pilonidal sinus develops when hair becomes trapped in or penetrates the skin, triggering inflammation that may lead to infection. It may remain unnoticed until pain, swelling or discharge develops.

Why Is Pilonidal Sinus Common in Young Adults?

Pilonidal sinus often affects adolescents and young adults. Several factors can contribute:

  • Prolonged sitting: Long hours studying, gaming, driving or working at a desk can increase pressure and friction around the affected area.

  • Hair and skin friction: Loose or coarse hair may penetrate the skin, contributing to inflammation.

  • Body weight and anatomy: Excess weight can increase skin friction in some people.

  • Other risk factors: Family history and certain hair-growth patterns may also play a role.

This condition is not simply a sign of poor hygiene, and it can affect people of different backgrounds.

Symptoms You Should Not Ignore

Watch for a tender lump near the tailbone, pain while sitting, redness, swelling, or pus or blood draining from the area. Fever or worsening pain may indicate a more serious infection.

If the lump is painful, swollen or discharging fluid, seek prompt medical assessment. Do not squeeze it or attempt to drain it yourself.

Can Pilonidal Sinus Be Treated?

Treatment depends on the symptoms and whether infection is present. Some cases without infection may only need observation and advice. A painful abscess may require drainage, while recurrent or persistent disease may need further treatment or surgery.

Keeping the area clean, taking breaks from prolonged sitting and reducing friction may be helpful. No preventive measure guarantees that the condition will not develop or return.
